Accessing charge slips
Please follow the steps below to create and sign your charge slip
- After signing your note, a charge slip will automatically open up with the pre-populated CPT code and diagnosis code.
- Ensure the CPT and diagnosis codes are correct in the Charge Slip Summary.
- Click Sign again > enter sign off password 1234 > Sign
Important!
Notes and charge slips will not save automatically until they’ve been signed. If you do not finish your note but are not ready to sign it, save your work by clicking Save at the top of the page before closing out the note or charge slip. Your note and charge slip must be submitted within 48 hours of the appointment.
Error message when signing a charge slip
You'll not be able to sign charge slips without a linked, signed note. If you try to sign a charge slip that doesn’t have a signed note, you'll get this error message:
Opening a charge slip
The charge slip is always linked to the session and can be located on the timeline in the patient’s chart. Click the encounter date to open the charge slip.
Link signed note to a charge slip
If you have a signed note for the session that needs to be linked, please look at the note on the timeline and drag it under the correct session. Double-check that you are filling out a charge slip on the proper visit.
- Find the right note on the timeline.
- Press and hold the note, then release it. The note will now become draggable.
- Drag the note under the session.
Create a new note that is linked to a charge slip
If you did not create a note for the session that you are billing with a charge slip, link a new note to the charge slip by clicking Add Linked and then Patient Note. Once you complete and sign the note, you’ll be able to sign the charge slip.
Adding a charge slip to a canceled appointment
You'll not be able to sign a charge slip if an appointment has already been canceled. If you have canceled an appointment without signing the charge slip, you can now un-cancel appointments to complete and sign the charge slip.
To un-cancel an appointment:
- Log into AMD (EHR or PM)
- Navigate to your Scheduler

Click on the list icon at the top left of your Scheduler
- Filter for the day of the appointment that you are wanting to un-cancel

Locate the appointment and un-check the box in the cancel column

Confirm that you would like to undo the action

Return to your calendar view and you should now see the appointment back on the calendar
- Now you are able to sign the charge slip associated with this appointment!
Other tips
Confirm that ICD 10 codes are correct and have populated the “Billing” section; this includes both the top and bottom of the ICD Code Search Tool.
Example:
Sign your charge slip when you’re done editing it using sign-off code “1234”.
