Chart review process

Overview

Chart review is an integral part of clinical practice and quality improvement at Rula. Through chart review, we can identify opportunities to enhance patient care and improve the quality and accuracy of documentation. This process applies to all Psychiatric Service Providers at Rula. A “reviewer” can be a Collaborative MD, peer, or other Rula clinical leader.

  • We will conduct chart reviews within the first 60 days or 1-2 randomly selected patient charts.
    • If any concerns arise (e.g., clinical concerns, missing multiple aspects of the below chart review template), the provider will be notified and a more thorough peer review will be conducted.
