Often, a medication is "re-trialed," and if it, or any variation of that medication, is on the discontinued list, it will be challenging to send a new prescription. Sometimes medications are incorrectly marked discontinued and need to be quickly and easily re-added to the active medication list. Occasionally medications are accidentally marked discontinued due to an error within the EHR.
Guided steps
When any of the above happens, follow this process:
- Open the Medications tab on the patient’s chart
- Use the dropdown that defaults to Current Meds to navigate to the Discontinued Meds list
- Check off the medication you want to take off of the Discontinued list
- Click Discontinue, which will open a pop-up
- Click Undo and then exit the window using the X button in the upper right corner. Do not click Done - clicking Done will keep the med on the Discontinued list.
- Close and re-open the medication tab. The medication will no longer appear under the Discontinued list.
