Patient education information and handouts

This article contains a list of patient education information & handouts. Providers should check UpToDate for more current information. These documents contain information on why providers may prescribe these specific medications, detail possible side effects, highlight the importance of taking medications as prescribed, and list circumstances when a provider may STOP prescribing them.

To share these with your patients:

1. Access the patient's chart through AMD
2. Go to Documents
3. Click plus sign to create a new Text Document
   
   
   
4. From the Template drop-down, select BZO or Stimulant med contact
   
   
   
   
5. Go over the med contract with the patient during your session
6. Click Sign
7. Send a message in AMD via VA distribution list asking the VAs to send the medication contract to the patient.
