In this article, we will cover how session notes can be created from a client's chart, located in the Clients tab of the Rula provider portal. Session notes include:
- Initial Assessment
- Progress Note
Guided Steps
To create a session note for an appointment:
- Log into the provider portal
- Click on the Clients tab, located on the menu on the left side of the page
- Select the patient you need to create the note for.
- Find the appointment card for the desired date, and click Start progress note
- This will say Start initial assessment if this is the first appointment with a patient.
- Please note: For the patient's safety, you should always confirm the patient's physical address at each session. Be sure to add this to your documentation as "Home" or "Address of File".
- Fill out the note, all fields are required.
- On the right side of the note, whether initial assessment or progress note, you will see a section called "Session reminders". This includes items such as a link to the patient's Measures, their next appointment date, and a quick link to the patient's safety plan.
- If you need to finish the note later, click Save & close
- To finish the note, find the appointment card in the client chart and click Complete progress note.
Note: This wording will reflect the template or appointment type (progress note or Initial Assessment).
- On the right side of the note, whether initial assessment or progress note, you will see a section called "Session reminders". This includes items such as a link to the patient's Measures, their next appointment date, and a quick link to the patient's safety plan.
- You'll see prefilled Zoom times for the start and end of your session with your patient. You can read more about that here.
- Once complete, click Sign note.
- Confirm your electronic signature, then click Sign and submit note.
- If you selected that you will not be continuing care with this patient in the "Next Steps" section of your note, you will be taken to a discharge note to complete.
Once the note is submitted, it cannot be edited. The signed note will generate a charge for the session and be billed to your patient and/or their insurance. A separate charge slip is not required for notes in the provider portal.
You will see a $ reflected on the note to indicate a charge slip has been completed—this usually takes 1–2 hours to appear after signing a note, as we process the charge slip. Please note that a delay does not indicate an error or missing charge slip, unless this icon has not appeared after 24 hours.
Diagnosis codes will carry over to future session notes and can be edited or adjusted as needed.
