Overview of clinical documentation at Rula

The Rula provider portal offers a range of documentation templates designed to streamline your clinical workflow. This article provides an overview of these templates, focusing on accessibility and categorization, to enhance your experience with initial assessments, progress notes, and other clinical documentation needs.

Accessing Documentation Templates

To access a new progress note in the Rula provider portal, you can do so from the client chart via the Client tab. Once on the client's chart, you will navigate to Notes & Documentation. From here, you will click Add note on the right-hand side of the screen. 

Categories of Templates: Billable and Non-Billable

Documentation templates in the provider portal are divided into two main categories: billable and non-billable.

Billable Notes

  • Initial Assessment and Treatment Plan Template: This is crucial for your first meeting with a new patient. It consists of six tabs, including History, Child/Adolescent (for minors), Risk Assessment, Mental Status Exam (MSE), Summary, and Treatment Plan.
  • Progress Note Formats: There are two formats available, each with three tabs: the progress note, a risk assessment, and the treatment plan. Changes made to the treatment plan will be reflected in future progress notes with that patient.

Each billable note, available in individual and couples/family versions, will automatically generate a bill for the patient(s) once completed and signed. You will see this symbol, $, to know the patient was charged.

Non-Billable Notes

  • Discharge Note: Used to indicate a patient's discharge from care or service termination. It's important to note that a progress note is still required for the patient's last appointment, despite the non-billable nature of the discharge note.
  • Blank Note: This flexible template can be used as an addendum to a signed note or for documenting any non-billable information.
  • Safety Plan: This template is essential for patients assessed as medium or high risk. It assists in creating and documenting a safety plan collaboratively with the patient during a session.

Steps to Ensure Payment:

  1. Complete the Note: After each session, fill out the session note thoroughly. Include all necessary details about the session, such as the date, time, and summary of the services provided. For missed appointment notes, this documents that the patient did not show for their appointment.
  2. Sign the Note: Once the session note is complete, make sure to sign it. Your signature verifies that the session occurred and that the details in the note are accurate. 

To ensure you receive payment, please review any unfinished or past-due notes. You can easily view these by navigating to the To-do tab of the provider portal, located on the menu on the left side of the page.
